Application Instructions for
Licensed Athletic Trainer (L.A.T.)
Specific requirements are detailed in the Rules and Regulations which are posted on this web page. You must review the rules in order to complete the required Jurisprudence Exam.
GENERAL INFORMATION
- In compliance with U.S. Code Title 8, Chapter 14, Section 1621, the Board must collect proof of U.S. citizenship, or proof of legal presence in the U.S. This proof is required only once for each U.S. citizen. Those with alien status will be required to submit proof with every renewal. In most cases it is easiest for you to provide us with a copy of your birth certificate, copy of your social security card or a copy of your passport. Just one (1) form of identification is required. A copy of your driver’s license is not acceptable. If you cannot provide any of these items, please refer to List A and B.
- The Board cannot review your qualifications without receipt of an application and fee.
- All fees are non-refundable. Make your check or money order payable to the “State of Wyoming”.
- Every effort will be made to review your application within two (2) to five (5) business days of completion.
- BOC stands for Board of Certification for Athletic Training.
REQUIREMENTSTo qualify for licensure as an Athletic Trainer (L.A.T.) in Wyoming you must meet the following requirements:
- be at least eighteen (18) years of age, and
- are a legal inhabitant of the United States, and
- have no felony convictions, and no misdemeanor convictions involving moral turpitude. (Exceptions to this requirement may be granted by the Board if consistent with the public interest.) and,
- have successfully completed an entry-level athletic training education program accredited by the Commission on Accreditation of Athletic Training Education (CAATE), or its successor agency, or have completed the National Athletic Trainers Association (NATA) internship program route, and
- have passed the BOC exam, and
- have passed the Board’s Jurisprudence Exam.
LICENSE APPLICATION PROCEDURES1. Print the Application for Licensure form, Supervision Agreement form, and Jurisprudence Exam Answer Sheet.
2. Complete the application in its entirety. Make sure that you provide a written explanation if you answered "YES" to any of questions 13 through 21. If more space is needed, attach additional sheets of paper. You must include copies of all legal documents. Read the “Warning” and “Agreement and Affidavit”, then sign and date the application, preferably with blue ink.
3. Complete the Supervision Agreement form. It must be signed by both you and your supervising physician, again preferably with blue ink.
4. Complete the Jurisprudence Exam by marking your answers clearly on the answer sheet.
5. Request that the BOC send the Board an official verification (written $25.00 or electronic $15.00 payable to the BOC) of your certification by contacting the BOC at http://www.bocatc.org/.
6. Mail the complete application (3 pages), proof of U.S. citizenship or legal presence in the United States, Supervision Agreement, Jurisprudence Answer Sheet, and the $600.00 application fee, to the Board at the address provided below.
PROCESSIf you have provided a valid e-mail address on your application, you will receive a confirmation message when your application, proof of U.S. citizenship or legal presence in the United States, Supervision Agreement, answer sheet and fee are received. When verification is received from the BOC, your application will be reviewed for license approval. If your application is approved you will receive a wall certificate, pocket identification card, and receipt.
