You must complete a Consumer Complaint Form and mail it to:

The Board is charged with investigating and acting upon complaints against licensed physical therapists and physical therapist assistants. All complaints regarding physical therapy care are taken seriously by the Board.
The Board will not consider a complaint unless it is signed and dated. All blanks should be filled in as completely as possible. Where the information requested is not known, please state this.
Be aware that if this complaint proceeds to the formal hearing level you may be called to testify as to your knowledge in the matter.
